create_entity
Create a typed memory entity (optionally materialized as a vault .md note). SQLite is the source of truth. Each string in observations must be a single non-blank fact with no embedded newline — a caller with more than one fact passes more than one array element. The result lists each observation with its observation_id; to correct or retire a fact later, pass that id to update_observation (an observation needs no key to be edited). Domain: knowledge.
